MEMBER SECURITY PROCESS

New Member Security Process
a. Usual paperwork for new member: PLUS the following:
(1) Two (2) Fingerprint Cards (FB-258 APPLICANT) Must have DCCG00100 US COAST GUARD CHESAPEAKE VA in the ORI Block; Dist 13’s DIRAUX address in the Employer block and “For Law Enforcement Purposes Only” in the Reason Fingerprinted block.
NOTE: (1) You no longer have to put your mother’s maiden name on the reverse side of the cards. (2) Do not put US Coast Guard Auxiliary in Armed forces NO: MNU, is for past military service numbers.
(2) OFI Form 86C (Special Agreement Check (SAC). (1 copy Original). New member fills out all applicable questions with exception of 7,8,9,10,11,12 & 14 (this is for the SECCEN) Leave them blank.
NOTE: The SECCEN signs in Block 14 as the Requesting Official there is no requirement for the member to sign the form. Leave block 14 blank.NOTE: requirement that Mother’s full name, Father’s full name & Mother’s maiden name is provided in 13 b. on the new OFI 86C.
(a) American citizens (by birth) need to fill out 1 through 6 and
13 a, & 13 b.
(b) Naturalized citizens need to fill out 1 through 6 and 13 a, 13
b & 13 c.
(c) Dual citizens need to fill out 1 through 6 and 13 a, 13 b, &
13 d.
(3) Standard Form 85- Authorization for Release of
Information. (1 Copy Original)
b. Auxiliary Personnel Security Questionnaire (PSQ). Disregard the “Due to your District Director of Auxiliary by 15 Jan 2004. If a new member wants Direct Operational positions, check Block 9- yes. A yes in block 9 covers both Direct Operational & Operational Support.
c. NOTE All new members under go the same PSI as Operational Support members. Option can be changed when they qualify for a Direct Operational position.
d. Send the Complete New Member Packet (a-b above) to the DIRAUX in Seattle.
Operational Support Members
a.
(1) Auxiliary Personnel Security Questionnaire (PSQ) if not done: Send in ASAP. Note: Auxiliarist will be automatically dropped from AUXDATA on 31 Dec 2004 if not sent in.
(2) Two (2) Fingerprint Cards (FB-258 APPLICANT) Must have DCCG00100 US COAST GUARD CHESAPEAKE VA in the ORI Block; Dist 13’s DIRAUX address in the Employer block and “For Law Enforcement Purposes Only” in the Reason Fingerprinted block. NOTE: You no longer have to put your mother’s maiden name on the reverse side of the FP cards.
(3) OFI Form 86C (Special Agreement Check (SAC). (1 Copy Orginal) Auxiliarist fills out all applicable questions with exception of 7,8,9,10,11,12 & 14 (this is for the SECCEN) NOTE: The SECCEN signs in block 14 as the Requesting Official, there is no requirement for the member to sign the form. Leave block 14 blank. Requirement that Mother’s full name, Father’s full name & Mother’s maiden name is provided in 13 b. on the new OFI 86C.

(4) Standard Form 85- Authorization For Release of Information (1 Copy Original) This form is used by investigators when requesting information about a person’s background.

Direct Operational Members

b. Three (3) Fingerprint Cards (FB-285 Applicant) must have DCCG00100 US COAST GUARD CHESAPEAKE VA in the ORI block; Dist 13 DIRAUX address in the employer block and “For Law Enforcement Purposes Only” in the Reason Fingerprinted block.
c. TWO (2) copies (Original & 1 copy) of SF 86- Questionnaire for National Security Positions with original signatures. (signed in black ink) (SF-86 includes the SF 86 Authorization for Release of Information and SF86 Authorization for Release of Medical Information-- Medical Release required ONLY if you answered “YES” to question 21. Note; Do not print the SF 86 release forms on the back of page 9 of the SF86 Questionnaire . Sign but, DO NOT DATE THE SF-86 or RELEASE SF-86 FORMS
d. TWO (2) copies of DHS Form 110000-9 (10-03) Disclosure and Authorization Pertaining to Consumer Reports Pursuant to the Fair Credit Reporting Act, with original signatures. (Replaces the DOT Form 1631) Sign but DO NOT DATE.
NOTE: DO members do not need to submit a SF 86 Certification. The SF 86C is a document that allows the reporting of changes on a previously executed SF-86 has changed. IF required will allow the individual to indicate that there has been or has not been any changes in the data on the most recently filed SF-86.
SF 86 Packet (3 fingerprint cards, 2 copies SF-86 with copies of SF-86 Release forms and 2 copies DHS-11000-9)
When members have their packets ready and they have been reviewed/checked send them to:

NOTE: FINGERPRINT CARDS: For race code use: “1” for American Indian; “W” for White, Hispanic/Latino; “A” for Asian; “B” for Black and “U” for unknown. No other information will be written on the reverse side of the card.
Your NO (OCA);FBI NO (FBI); Armed Forces NO (MNU); and Miscellaneous NO (MNU) on the front of the card are discussed on the back of the Card under Instructions # 4, if you know them put them in the correct block, if not leave blank.
On Fingerprint cards in the reason fingerprinted block use: “For Law Enforcement Purposes Only”.
On SF-86, All addresses must be a physical street address, can not use a Post Office (PO) box address.
Added: Make sure you use the correct OFI Form 86C. The new one has blocks for your mother’s full name, mother’s maiden name and father’s full name in block 13 b.
On the Fingerprint card: DO NOT PUT USCG AUX in the Armed Forces NO MNU block. This is for military service numbers only. There is no place or requirement for Auxiliary on the card. Make sure cards are dated and signed by both the person fingerprinted and the person taking finger prints.
NOTE: IF YOU CAN’T READ THE PRINTS AS SHOWN ON THE BACK OF THE CARDS (The lines running between the loops and deltas or between deltas must be clear) THEY WILL NOT BE ACCEPTED. Make sure cards are dated and signed.
